Can I book physiotherapy in Ontario without a referral?

Yes. Physiotherapists in Ontario have been primary contact practitioners since 1994, which means you are allowed to come to us first, the same way you would book a dentist. Nothing in the law or in our college rules requires a doctor's note before an assessment.

People are often surprised by this. Every week someone calls our Kitchener clinic and opens with an apology: they have not seen their doctor yet, is that okay? It is more than okay. Part of a physiotherapy assessment is screening for the situations where you do need a doctor, and if I find one, I will tell you plainly and send a note your doctor can act on.

Why do some insurance plans still ask for a referral?

Because plan contracts are written by insurers, not by the province. Your right to book directly and your plan's rules about reimbursement are two separate things. Some extended health plans reimburse physiotherapy with no questions asked. Others will only reimburse if a doctor's referral sits on file, and a few want the referral renewed every year.

The frustrating part is that two people working for the same employer can have different rules, depending on which plan tier they chose. So nobody can answer this for you from the outside, including us.

How do I check what my plan requires?

Call the number on the back of your benefits card and ask two questions: does my plan cover physiotherapy, and does it require a doctor's referral for reimbursement? Five minutes on the phone saves a rejected claim later.

If phone menus are not your thing, bring your insurance details to your first visit and we will help you check. We do this daily, and we would rather sort it before your first claim than after.

What about WSIB and motor vehicle accident claims?

These run on their own paperwork, separate from extended health benefits. You still do not need a doctor's referral to be assessed, but the claim itself has forms, and treatment is billed through the insurer or WSIB rather than your benefits plan. Bring your claim number and the date of the incident to your first visit and we handle the forms from there.

What does physiotherapy cost if I pay out of pocket?

At our clinic, an initial assessment costs $110 and follow-up sessions cost $85, as of May 2026. You will see those numbers in writing before anything starts, because surprise charges are not something we do. If you have extended health benefits, physiotherapy is often covered in part or in full, and we bill most major insurers directly.

So what should I actually do?

If something hurts, book the assessment. The referral question sorts itself out afterwards: either your plan does not need one, or you ask your doctor for a note before your first claim goes in. What tends not to sort itself out is the thing that made you search for this article. Our back pain page explains how an assessment works for the most common complaint we see, and the services section covers everything else the clinic offers.
